- Industry-Submitted Incident Notification: Dropped Blocks: A service rig was doing a workover, running rods, when the rod transfer cable became stretched and frayed. A Driller and Derrickhand stopped to replace the frayed rod transfer cable. During the replacement, the Driller chained the brake handle but did not engage the disc brake. The brakes cooled and released, causing blocks to fall uncontrolled to the ground.
